Mara is in labor with twins when she calls her husband Franco, a renowned gynecologist, for help. He coldly tells her he is performing a C-section on Leanna's dog, which carries eight puppies, then hangs up and blocks her number. Their young son Alec runs to find him, but Franco shoves the boy aside while rushing the dog to a pet hospital; Alec is struck and killed by a truck. The twins die in Mara's womb, and she loses the ability to bear children.

After emergency surgery, Franco barges in with Leanna, mocking Mara and demanding her breast milk for the puppies. When Leanna slashes her wrist, Franco accidentally cuts Mara's thigh and kicks away the box holding their dead twins' bodies. Later he returns with a hollow apology, then leaves again for Leanna.
